Responsiblefor:
  • Intake process and oversight
    • Participate in and manage the new patient intake workflow from pending status to active status which includes:
      • Initial communication with patient and/or family members regarding services, rates, and community resources.
      • Able to determine patient needs and make recommendations on this basis.
      • Proper completion of new intake in EMR
      • Distributing Patient Information to the pertinent departments and/or individuals via EMR workflow and inbox and other supporting communication means such as Teams.
      • Communicating with appropriate office personnel (i.e., Director of Business Ops, Director of Clinical Services, Client Services Associate)
      • Oversees sending signature paperwork or company information, either by mail, email, or fax to inquiring patients or other responsible party
      • Oversee communication with family or responsible party routinely until case becomes active or moves to a non-admitted status.
      • Oversee communication with the referral source contact (care manager, doctor’s office, discharge planner, etc.) until case becomes active or moved tonon-admittedstatus.
      • Daily oversight of on-call report, identifying situations that are appropriate for referrals/intake.
      • Ensure Patient Information Sheets are correct and updated in EMR.
      • Obtain initial Physician orders on new Skilled Nursing cases.
      • Keep local offices, billing department, the authorization department, and other appropriate departments informed of intakes, information calls, and non- admits.
      • Prepare weekly report on new starts, new referrals, discharges and holds every Friday via EMR report and/or Power BI.
      • Cultivate and develop relationships with appropriate referrals sources including but not limited to care managers of Medicaid Managed Plans, physician offices, hospital discharge planners, social workers
      • Maintains a current Referral Source Spreadsheet
      • Communicate any patient’s concerns to the appropriate Director of Clinical Services or Director of Business Ops.
  • Authorizationresponsibilities:
    • Perform or oversee insurance verification process on new patients prior to acceptance of new cases
    • Obtaining proper authorization for new patients, ensuring the local offices are aware of amount and type of authorized services.
    • Ensuring updated authorizations are obtained in timely manner.
    • Oversee that authorizations are attached in Admission documents in EMR and properly entered into EMR and that correct billing is set up in the EMR.
    • Communicate with local offices and Director of Clinical Services if the Health Plan does not authorize service. Work with the local office to decide on non-admission due to no identifiable payer source.
    • Obtain Single Case Agreements as needed, collaborating with Director of Contractual Development or Director of Business Operations
  • Other responsibilities:
    • Maintains awareness of and compliance with state, accreditation and federal guidelines and requirements.
    • Attends community in-service, seminars, or other events as directed.
    • Attend departmental meetings as required.
    • Schedules and hosts routine Intake Department meetings.
    • Ensure department is aware of budget to goal results.
    • Promote good teamwork by working with all departments in friendly, cooperative environment.
    • Monitor, manage and approve Client Success Department staff payroll and time off.
    • Conduct annual performance evaluations for Client Success Department.
    • Ability to communicate verbally and through written words in a professional manner.
    • Treat and communicate with patients, families, co-workers, referral sources and caregivers with courtesy, respect, and consideration.
    • Maintain confidentiality of all patients, caregivers, and business information.
    • Comply with all applicable laws, regulations, policies, procedures, and guidelines.
    • Report to work as assigned or scheduled. Follow procedures for notification of absence or time-off requests.
    • Maintain professional/technical competency and proficiency of job responsibilities by attending and participating in meetings, conferences and education programs as requested.
    • Perform other related duties as needed and requested by Supervisor.
    • Perform all job responsibilities with a friendly, positive, and collaborative attitude.

Qualifications:
  • Bachelor’s degree (preferred)
  • Five years home health care experience
  • Pediatric home health experience (preferred)
  • Management experience
  • Experience working with Medicaid Eligibility and Authorization process

Annual Salary Range:
  • Full-Time, $70,000 - $85,000.00/year
*Compensation dependent on experience.

Location: Delaware, New Jersey or Pennsylvania
